Alivio continues to offer COVID-19, flu and monkeypox vaccines.

  • Morgan/Western/Berwyn
    • ​Mon/Tues/Thurs/Fri: 9am-12pm & 2-5pm
    • Wednesdays: 1-3pm & 5-7pm
    • Saturdays: 9am - 12:30pm

How do I schedule a vaccine appointment?

There are currently 3 scheduling options:

  1. Book your appointment online by clicking the button below (if using a desktop/laptop, use the Chrome browser):


     

  2. Text the keyword corresponding to the type of vaccination you want and a representative will call you within 24-48 hours to verify eligibility and schedule your appointment. Text the keyword to 773-305-6064:

  3. By phone at 773-254-1400. Please be patient as our representatives are experiencing high call volume.
